A U.S. citizen may receive a full validity (ten year) passport beginning at age 16. Applications for these passports are accepted by appointment. The applicant must appear in person.
In preparation for your upcoming appointment, please take note of new requirements for passport applications.
We will no longer accept applications that are handwritten, your application must be generated through the link below and printed with a barcode strip to the left side of the application.

Check your local embassy’s website on how to make an appointment, or in some cases, drop off your passport application. Walk-in services may not be available. Passport services are not available on weekends or on The Bahamas or U.S. holidays.

Applicants Age 16 or Older (DS-11)
You should complete this form if you’re applying for the first time, or you don’t meet our requirements to renew your passport.
Applicants Age 15 or Under (DS-11)
You should complete this form if you’re applying for your child who is under age 16.
Applicants Age 15 or Under Without Both Parents Present
You should complete this form if one parent or guardian cannot go with the child to apply in person for the child’s passport. The parent or guardian should submit this special form in addition to their child’s application (DS-11).
Adult Passport Renewal (DS-82)
You should complete this form if you meet our requirements to renew your passport. Keep in mind that children under age 16 cannot renew their passports and must apply using Form DS-11.
Report and Replace Lost/Stolen Passport (DS-64 & DS-11)
You should complete this form if you lost your passport or it has been stolen. To replace a lost or stolen passport, you must apply in person and submit Form DS-11 in addition to Form DS-64.
Update Current Passport or Replace Emergency Passport
You may be eligible to use this form if you need to change or correct your passport, or you need to replace a passport that was limited to less than 10 years validity (which is the normal validity period for an adult’s passport).

You must provide one photo with your passport application. All our photo policies apply to both adults and children under age 16. We have a few exceptions to our photo policies for infants.
The U.S. Embassy in Nassau has the ability to issue emergency U.S. passports in certain limited circumstances. These passports have a restricted validity and are generally issued only to enable stranded U.S. citizens to return home. Persons who need emergency passports must apply in person between 8am-11am on business days (excluding local and U.S. holidays) at U.S. Embassy Nassau. No appointment is required, although the applicant should present a police report (if their passport was lost/stolen) and an itinerary showing urgent travel within the next two weeks.
